Please tell us how you will access this site by selecting your User Type.

The secure portal will provide information and materials for review.
Once registered, information accessed will be: Eligibility, Benefit Details, Claims and Notice of Authorization.
To register for online account access, follow the steps below.

Select Provider from the User Type drop down menu, then press next.
Follow the prompts, enter your Office Tax ID and Physical Zip Code.
Enter registration information by following the prompts; User First Name, User Last Name, Email.
Please save your new “Username” and “Password” for future use.
